Patient Information



You should call the office at least three (3) days before an appointment to provide information necessary to complete a referral. That information should include: your name; physician being referred to; physician phone and fax numbers; date of  appointment; referral diagnosis; insurance information; whether it is for a visit or a specific procedure (such as surgery); and where any procedure will be performed (office or hospital). We do not mail referrals to specialists. Be familiar with your insurance plan and determine if it requires a referral to another physician.  If uncertain of how a referral is handled, please call and ask our staff.


Lab and X-Ray Results


Please refer to our Lab Results Policy in this packet. When lab or radiology work is ordered by another physician other than BCS Pain Specialists, please give us two (2) days to complete this request. You can call or drop off the order.


Emergencies and On Call


If you have an emergency such as chest pain, severe shortness of breath, severe headache, or bleeding, call 911 or proceed directly to the nearest hospital emergency room. You can always call our office with any questions about acute non-emergency medical problems after hours. If the physician is not in the office, an answering system will forward a page to the doctor.  If the doctor on call does not answer your call within 30 minutes, please recall the office.


Records Release


Please complete the enclosed records release to obtain medical records from your previous family physician and any consulting specialists. Our Practice’s Privacy Policy is available at the office for your review.
